Team Coach Waiver (version 1)
This waiver and release of liability form is intended to inform team coaches of the potential risks associated with bringing a team to youth basketball clinics, games, and tournaments hosted by 127 SPORTS Intensity, and to confirm the coach's responsibilities for the players on their team. Please read each section carefully and sign at the bottom to acknowledge and accept the terms outlined below.
1. Heads Up Concussion Waiver
In compliance with the Heads Up Concussion initiative, I acknowledge that I have received, read, and understand the signs and symptoms of concussions. I agree to remove any player from play immediately if a concussion is suspected, to notify clinic staff and the player's parent or guardian, and to keep the player out of activity until a licensed healthcare professional authorizes their return.
2. Injury and Liability Waiver
I understand and acknowledge that participation in youth basketball activities carries inherent risks of injury, including, but not limited to, sprains, fractures, dislocations, and head injuries, both to players and to coaches. I voluntarily assume these risks for myself. I hereby release, discharge, and hold harmless 127 SPORTS Intensity, its coaches, staff, volunteers, and affiliated parties from any claims, demands, or liabilities arising from injuries I sustain during any clinic or club-sponsored activities. This release covers all injuries or damages, whether they are physical, emotional, or mental, and whether resulting from negligence or other causes.
3. Parent and Guardian Consent for Players
I confirm that I have informed the parent or guardian of every player on my team about this event, its date and location, and the risks described in this waiver, and that each parent or guardian has given permission for their child to participate. I understand that 127 SPORTS Intensity may require a separate waiver signed by each player's parent or guardian, and I agree to collect and provide those waivers if asked.
4. Player Insurance and Medical Information
I understand that 127 SPORTS Intensity does not provide personal health or accident insurance for players or coaches. I confirm that I have informed each player's parent or guardian that their child must be covered by their own health insurance. I agree to have emergency contact information for every player on my team available at the event.
5. Video, Photography, and Publication Waiver
I grant permission to 127 SPORTS Intensity to capture my image, voice, and likeness, and that of my team, through video, photography, or audio recordings during clinics, games, tournaments, and other related activities. I confirm that I have told each player's parent or guardian that photos and video may be taken and used for promotional and training purposes, including social media, club websites, printed materials, and media publications. I will tell clinic staff before the event about any player whose parent or guardian has asked that their child not be photographed.
6. Medical Authorization and Emergency Response
In the event of an emergency involving me or a player on my team, I authorize 127 SPORTS Intensity staff to contact emergency services and to assist in securing medical treatment. I agree to cooperate with staff in contacting the player's parent or guardian as quickly as possible. I assume financial responsibility for any costs incurred as a result of emergency medical treatment I receive.
7. Coach Code of Conduct and Team Supervision
I acknowledge that I, my players, and the spectators who attend with my team are expected to abide by the 127 SPORTS Intensity Code of Conduct, which promotes sportsmanship, respect for coaches, officials, teammates, and opponents, and adherence to club rules. Because this clinic trains youth referees, I agree to model respectful behavior toward all officials. I am responsible for supervising my players at all times while they are at the event, including before and after their games. I understand that failure to comply may result in my team's removal from the event.
8. Transportation Waiver
I understand that transportation of my players to and from the event is arranged by me, my team, or the players' parents and guardians, and not by 127 SPORTS Intensity. I assume all risks associated with that transportation and release 127 SPORTS Intensity from liability for any incident or injury occurring during transportation.
Summary and Consent
By signing below, I confirm that I have read, understood, and agreed to each waiver section above. I recognize and accept the risks involved in youth basketball activities with 127 SPORTS Intensity, I accept responsibility for supervising my team, and I agree to the terms outlined above.
Acknowledgment of Entire Agreement
I certify that I am at least 18 years old, that I am the coach responsible for the team named in this registration, that I have read this waiver, understand its content, and agree to all terms as stated. I understand that this waiver represents the entire agreement between myself and 127 SPORTS Intensity, and no other verbal or written statements will alter its terms.
